Specifically, what ones should I get? I generally play as an inaccurate TD or just general support. Typically, I'll wait around at the spawn and then just go where I'm needed. Scout heading for arty? Friendlies pinned down? An enemy that isn't dead? I zip in. Murder. And then zip away. I also try to take advantage of sloped terrain whenever I can. It goes just as fast horizontally at 45 degrees as it does at 0.
Lastly, not sure if this matters much, but I've been using the 85 mm HE shells lately. At first, I tried the 57 mm ZiS-4 after finding the 85 mm's aim time and accuracy kinda lacking. It has near identical penetration and does about half the damage at two times the fire rate. Not only that, but it aimed fast and was more accurate. However, this meant it took longer to kill stuff. Not much longer mind you, but enough that enemies were getting shots they otherwise wouldn't have if I'd been sporting the 85. And the T-28 is armored with paper-mache and prayers. So, it was back to the 85, and then I realized something: why not give myself a larger margin for error? I've been using HE rounds ever since and it's been working out really well.

-Commander: Recon (+view range) or Jack of all trades (replaces KO'd crew member)... jack of all trades might be good if your driver gets killed while you're trying to run away, but recon would be great for spotting danger BEFORE it kills you.
-Gunner: Snap shot (+accuracy while rotating turret), for the same reason as below!
-Driver: I'd definately go with Smooth Ride (+accuracy while on the move), since you're gonna be a hit-and-run type person
-Radio: Call for Vengeance, signal boosting, or situational awareness, 2 of these three for your radio operators... DON'T DUPLICATE, only one is worthwhile! -Radio
-Loader: Loader perks are pretty lackluster... camouflage is probably the best one for the loader, to stay hidden to prevent you from getting pwned.

Off Road is a good one for the Driver's position and Deadeye is good for the Gunner (but needs 100%). As for equipment, if your fire rate is under 15 rpm then I would go for the gun rammer, Binoculars and Cammo net (if you like to hide a lot)

Depends on the map or game type, but I'm usually on the move. Sitting still if the enemy sees me is pretty fatal. What would you suggest instead? Coated optics?
